Terms and conditions for the provision of healthcare at the practice of MDDr. Veronika Andělová, in force from 1 January 2018.
This is a translation provided for the convenience of patients. The Czech version of this document is the legally binding one.
These Terms & Conditions (hereinafter the "conditions") govern the contractual relationship between the provider of healthcare services and the patient – client (the recipient of healthcare services).
The provider of healthcare services is MDDr. Veronika Andělová, IČO: 04226496, hereinafter also referred to as the "provider".
The terms "treatment", "care", "procedure" and "treatment session" mean the provision of healthcare services within the meaning of the law.
The term "dentist" means a person acting on behalf of the provider, whether in the role of a dental practitioner or in the role of a dental hygienist.
The term "practice" means a complete operating unit providing a healthcare service, consisting of a dental unit (chair) with the corresponding technological, diagnostic and treatment equipment, including the dentist, the assistant and the auxiliary medical staff.
Obligations of the provider of healthcare services
MDDr. Veronika Andělová undertakes:
- to perform healthcare in the field of practical dentistry, oral surgery, aesthetic dentistry, etc. at the level of current modern medical knowledge, in accordance with the best interests of the patient's health and in accordance with the applicable regulations and laws of the Czech Republic;
- to keep the patient's medical records in accordance with the applicable regulations, in electronic form;
- to handle the information stored in the records in accordance with the principles of personal data protection and other statutory regulations.
Before every healthcare procedure, the patient will be informed by the dentist to a sufficient extent:
- about their oral health condition;
- about the proposed treatment procedure (treatment plan) as well as about the nature, consequences and possible risks associated with the given healthcare procedure;
- at the same time, they will be informed of the price paid by the patient for this procedure.
Where several possible treatment options exist, the treating dentist will acquaint the patient with the individual options and their cost.
MDDr. Veronika Andělová is obliged to book patients for the agreed time so as to minimise patients' waiting. At the same time, however, she reserves the right to determine the order of patients. In the event that the agreed treatment time is not kept by more than 30 minutes, the patient will be offered the option of a new appointment.
MDDr. Veronika Andělová is obliged to observe all the ethical principles of the dentist/patient relationship, to minimise the stress and any pain of the care provided and to communicate with the patient so that the patient understands everything.
MDDr. Veronika Andělová reserves the right to refuse or terminate treatment if:
- the patient does not follow the proposed treatment procedure;
- the patient repeatedly commits an unexcused absence – fails to keep the agreed appointment without timely notice;
- the patient does not abide by the provider's internal rules or restricts the rights of other patients;
- the patient's demands conflict with the dentist's professional conviction;
- for other statutory reasons (operational or capacity reasons).
Obligations of the patient
The patient is in particular obliged:
- to abide by the provider's internal rules;
- to inform the dentist truthfully about their health condition and related matters;
- to inform us without delay of any change of their health insurer. In the event that the patient fails to fulfil this obligation and the care provided is not reimbursed by the health insurer, the treatment provided and the care reported will be charged to the patient for direct payment in accordance with the practice's valid price list;
- to follow the proposed treatment procedure;
- to care for their oral health in an appropriate manner so as to avoid all removable (preventable) health risks and to maintain a high standard of their dental hygiene;
- to accept preventive check-ups and preventive dental hygiene as an essential part of treatment;
- to provide cooperation during the healthcare procedure and the monitoring of the course of the treatment process;
- to keep the agreed appointments for the individual treatment procedures in accordance with the treatment plan, the preventive check-ups and the dental hygiene procedure plan;
- after the completion of treatment, to attend regular preventive check-ups once every 6 months (unless the treating dentist determines otherwise), even if they have no problems. As part of the regular check-ups for adult patients, 3D scans of the jaws are produced, among other things, which serve as an aid to diagnosis and are paid for by the patient on the day of scanning, at a value of 800 Kč.
The agreed appointment is deemed to be the appointment recorded in the provider's system, and the provider must communicate it to the patient – orally, by telephone or by email. The provider is entitled to change appointments for operational reasons and to inform the patient (orally, by telephone or by email) of such a change.
The patient is aware that in the event of their unexcused failure to keep an agreed appointment, or the cancellation of an agreed appointment less than 24 hours in advance for appointments of less than an hour and one-hour appointments, and for appointments of 2 or more hours less than 48 hours in advance (hereinafter the "absence"), the provider incurs damage proportionate to the practice's per-minute rate. The patient and the provider have agreed that in the event of the patient's absence, the provider is entitled to charge the patient damages in the amount corresponding to the cost of running the practice (i.e. the practice's per-minute rate) to the extent corresponding to the planned length of treatment. The damage quantified by the provider is payable on the day of the patient's absence and the provider is entitled to invoice it to the patient subsequently. The patient acknowledges that the purpose of this provision is not to impose a financial burden on them, but to prevent unexcused absences and idle time of a costly medical facility.
The patient's absence may be excused in advance and a replacement appointment arranged. In such a case, the notice must be given at the latest 24 hours in advance for one-hour appointments, and up to 48 hours in advance for appointments of 2 or more hours, in a demonstrable manner – by electronic mail to info@zubar-andelova.cz or by telephone on +420 778 760 057.
Furthermore, the patient is aware that in the case of a booking for a treatment appointment of 2 or more hours, a deposit of 5000 Kč must be paid before the treatment. After the treatment has been carried out, the deposit will be deducted from the total amount for the treatment on the day of the procedure. In the event that the patient does not attend the treatment, the deposit is forfeited. In the event of an unexpected change and the impossibility of attending the procedure, rebooking is possible at the latest 48 hours before the planned treatment appointment. In the event of illness and cancellation of the booking on the day of the procedure, a confirmation from a doctor is required for the deposit to be refunded. Otherwise, the deposit is non-refundable.
Premium dentistry
MDDr. Veronika Andělová offers premium dental procedures, the most modern technologies and minimally invasive (painless) procedures. Even in the area of premium care, several treatment options always exist and the patient is given a choice of several possibilities. The patient is always informed about the possible care options and their prices – before the treatment is provided – by means of a treatment and budget plan.
Contracted health insurers
MDDr. Veronika Andělová holds contracts with health insurers in the Czech Republic, except for RBP. The patient acknowledges that the care reimbursed by health insurers covers only certain basic types of treatment, and only using the most basic materials. For other treatments and treatments using higher-quality materials, the treatment will be charged to the patient for direct payment.
The patient is obliged to inform the dental practice without delay of any change of their health insurer. In the event that the patient fails to fulfil this obligation and the care provided is not reimbursed by the health insurer, the treatment provided and the care reported will be charged to the patient for direct payment in accordance with the practice's valid price list.
The price list of services is available for inspection at the practice or on the website www.zubar-andelova.cz.
The individual possible treatment options and a price proposal for the given patient will be drawn up by the dentist as a treatment and budget plan after payment of the amount of 1500 Kč. In the event of complications and greater time demands of the treatment, the planned price in the treatment and budget plan may ultimately be increased only in accordance with the practice's current per-minute rate. The stated budget plan may further change depending on the level of dental hygiene and the length of time between the creation of the treatment plan and the start of treatment, or on the development of the current ČNB exchange rate and the associated change in the prices of dental material in particular on the European market. Payment for treatment is made in cash or by payment card. In the case of external production of prosthetic products or the purchase of specific implants, a deposit payment is required, the amount of which is determined by the dentist, who informs the patient of this fact in advance.
The per-minute rate (MS), used to calculate the prices of procedures and products, represents all the costs of running the practice – including the professional work, the technologies and materials used and the provision of the practice's hygiene regime.
Special materials (single-use instruments, special compounds, etc.), implants and prosthetic products are not included in the per-minute rate and are charged to the patient separately.
Complaints
MDDr. Veronika Andělová provides a 2-year warranty (from the moment of completion) on prosthetic work (metal-ceramic / ceramic crowns and bridges).
The warranty does not apply to fillings, temporary work, plastic crowns, endodontic treatment, or dental procedures of the nature of services (dental hygiene, whitening, procedures on soft tissues, etc.).
The reasons for the total or partial cancellation of the warranty are in particular:
- the patient's negligence of oral hygiene;
- failure to heed the dentist's recommendation;
- improper use of a prosthesis;
- an untreated dysfunction of the jaw joint or an orthodontic defect;
- systemic diseases affecting the state of oral health (diabetes, epilepsy, osteoporosis, cytostatic treatment, etc.);
- injuries to the head and neck area with the possibility of injury to the teeth or jaw.
A complaint must be submitted by the patient primarily directly to the provider, who, as part of the register of complaints, must consider the matter and deal with it within the statutory time limit.
Final provisions
The contractual arrangements come into force upon entering the practice of MDDr. Veronika Andělová.
